Trinity medical care is a patient centered medical home (PCMH), where its provider aims to attain all the patients’ health care needs.

Trinity Medical Care provides the following services.

·         Continual access to a healthcare provider

·         Continual Care

·         Coordination of patient care

·         Preventative patient care

·         Complete Care

Trinity Medical requires its patients to recognize the following and take responsibility.

·         Provide the office with accurate medication lists, and other vital health care information.

·         Listen and follow the instructions given to you by the health care provider or staff of Trinity Medical Care.

·         Ask, if there is any information pertaining to your health care needs or medications that you have questions about or need better understanding.

As patients of Trinity Medical Care:

·         You may be asked to have your bloodwork done before your office visit.

·         You may participate in the creation of your care plan and provide a copy.

·         You may be asked to present your health goals to the provider, so we can help achieve those goals.
