The Advanced Care Team supports your Primary Care Provider in their efforts to deliver effective patient care.

What is a Co Management Agreement?

The co management agreement is between the Primary Care Provider and the behavioral health provider or urgent care. The purpose of this agreement is to address what the primary care provider and the behavioral health provider or urgent care is responsible for to make patient care as seamless as possible. This agreement is to establish efficient communication in both directions.

Trinity Medical Care was founded by Lloyd Jeffrey Taylor P.A.-C in 2010. This private practice focusses on wellness, acute and chronic care, adolescent medicine, adult care, and geriatrics for its patients.
